Drive Thru Flu Shots- Please select your desired location and time. Schedule for each individual that will receive a flu shot at the time of your appointment. All participants must be 3 years or older.
HRA or Health Risk Assessments- Please select the correct service and location from the drop downs on the left. Tips for a smooth screening: Please remember to fast for 8-12 hours prior to your appointment. Water and Black Coffee are premitted during this time. Being hydrated is important. Please take all maintenance medications as prescribed unless they are required to be taken with food.
Mobile Mammography- Please select your desired location and time. An Ascension Breast Center Representative will call you prior to your appointment to review your appointment time and procedure. Be sure to list the best contact number with your appoinment info. |
