On-line Scheduling System
 

Welcome to our on-line scheduling system. All students are screened by a Registered Nurse prior to being seen by the Nurse Practitioner or Physician.

 ALL appointments are FREE.  Tests are NOT.    There will be a $10 charge for "No Shows".  If you fail to show up for your appointment or don't call to cancel, your account will be charged $10.  Please show up for your scheduled appointments!

 A valid ID must be presented at check-in if requested.

 

Visit Types:

  • Medical Clinic (Monday - Friday): All general medical illnesses, physical exams, immunizations, lab tests
  • Women's & Men's Clinic (Thursday Only): All illnesses that pertain to sexual organs (STD testing, Birth Control, Paps)

Notices Please Read Before Scheduling:

The Women/Men's Health Clinic will resume in September. There WILL NOT be a Women/Men's Health Clinic during the summer.

 

Make sure if scheduling for Mens/Womens Health, you should be scheduling in the appropriate column (Far right). Look at headings.)


Health History Form: Complete and bring it with you to your appointment (found on the health center web page) or arrive 10 minutes early to complete prior to your appointment.

Please bring your insurance card with you.

 

TB Skin Tests: (48-72 hours after test is administered, you must return to clinic for the reading).  PPD Test can only be administered on Mondays, Tuesdays, and Fridays.

 

Directions for scheduling:Read all directions before beginning the scheduling process.
1. Select Service:
A. New patient--for students who have never been seen at the Health Center. .
B. Established patient--for students who have been seen in the health center.
2. Select Date
3. Select available Time.
4. If you are using the on-line scheduling system for the first time, choose new user and enter the required information.
5. Finalize appointment.
