Welcome to the LIHEAP online scheduling portal. If you are self-employed, please call 815-308-6057, leave your name, phone number and that you are self-employed. Someone will call to schedule your appointment. If you have a current disconnection notice, please call our office at 815-338-7752. You will need to provide the disconnection notice. Please review the required list of documents shown below.

PRIORITY PERIOD	WHO CAN APPLY?
OCTOBER	Households must have at least one of the following: 1. Have a member(s) that are 60 or over 2. Have a member that is disable and receices disability 3. Have child(ren) aged 5 and under. 4. For households that are disconnected or have received a disconnection notice. (You MUST be able to provide a notice)(PLEASE CALL 815-338-7752)
NOVEMBER	All households may apply
DECEMBER

All appointments will be telephone appointments. We will call you from an 815-308 phone number at your appointment time. Please be sure to answer your phone.
For the 2025 program year, households and individuals who have gross incomes that are less than the following maximums, for the last thirty days, may apply for LIHEAP.

Applicants must have the following documentation with them during their phone appointment:

1. Proof of all gross income for all household members, for the 30 days prior to the day of and including their appointment date (includes, but is not limited to: paycheck stubs, Social Security income, Unemployment, SSI, TANF, General Assistance from Township, pension,) **if you are self-employed, there are extra required documents**

2. Current Utility Bills -gas, electric, fuel oil, propane

3. Social Security Cards or ITIN for all household members. Residents without ITIN or SSN can still apply.

Note: Depending on circumstances, other documentation may be required

If you qualify, then you may continue on to make an appointment. Please allow 30-45 minutes, depending on your family size, for your telephone appointment.
