Please be prepared for your telephone appointment with the following items

Social Security Card numbers and birthdates. If you are new, you must supply a copy of the actual card
Proof of Wages
               Income received from Social Security
               VA Benefits
               TANF assistance
               General Cash Assistance
               Other income not listed here
Current Gas  Electric or propane billing statement
There may be other documents dependent on your particular household situation
