Scheduling Instructions 

1. IMPORTANT:  Select your time zone in the box below to convert the available appointment times to your time zone.
2. Select an available date on the calendar and then click "book it" to select your appointment time.
3. Enter the required fields (marked with an asterisk *)
   • To receive a text message reminder to your phone an hour before your apointment, enter your cell phone number.
4. Click, "Finalize Appointment".
5. An appointment confirmation with instructions will be sent to your email address.
