- Select an available date on the calendar and then click "book it" to select your appointment time.
- Enter the required fields (marked with an asterisk *)
- To receive a reminder via text message to your phone an hour before your apointment, enter your cell phone number.
- To receive a reminder via email, enter your email address.
- Click, "Finalize Appointment".
- At your appointment time, a Benefit Specialist will call you to discuss your benefit options, answer questions and enroll you in your benefits.
