IMPORTANT PROGRAM UPDATE
We are excited to announce we will be offering phone and video appointments for Health Insurance Enrollment during the 2021 Open Enrollment Period, which runs from November 1st until December 15th.
Please note that the following appointments are ONLY for assistance with completing our intake form, the first step of our remote process this season. After the intake form is complete, you will be able to set an enrollment appointment between November 1st and December 15th.
To Complete Your Intake Appointment, you will need to provide us with:
· Your Marketplace username and password
· Name, date of birth, health coverage information and immigration status information for applicant and tax household members
· Details and cost of health insurance offered by your/your spouse’s employer(s)
· If you had a Marketplace plan & financial help in 2019, whether you’ve completed your taxes for 2019, including the 8962 Reconciliation Form
