 PLEASE READ THE FOLLOWING INSTRUCTIONS 



1. Individuals who have previously received an initial series of any of the three COVID-19 vaccines currently approved or authorized for use in the United States are eligible for a booster dose.

 
2. At this time, WhidbeyHealth is not offering vaccine to persons under the age of 12.  

 

3. If you are starting your vaccine series, please only book your first appointment. We will assist you in booking your series completion appointment during your first appointment.  

4. Please bring your CDC COVID-19 Vaccination Record Card to your appointment along with state-issued identification (i.e., driver’s license, passport).

5. Please also print, complete and bring with you the attached WhidbeyHealth COVID 19 Consent Form.

Pfizer Info:

Comirnaty
® (COVID-19 Vaccine, mRNA) Pfizer-BioNTech Booster Dose may be administered to individuals:

  • 65 years of age and older

  • 18 through 64 years of age at high risk of severe COVID-19

  • 18 through 64 years of age whose frequent institutional or occupational exposure to SARS-CoV-2 puts them at high risk of serious complications of COVID-19 including severe COVID-19


A third/booster dose may be administered 
when it’s been at least 6 months after completion of your primary series.  Immunocompromised individuals may receive a third dose 28 days after their second dose.
 
 

Moderna Info:

Those who received the 
Moderna COVID-19 vaccine are now eligible to receive a booster dose at six months or more after their initial series. 

Among recipients of the Moderna COVID-19 vaccine, those who should receive a booster dose of COVID-19 vaccine include those at increased risk for hospitalization and severe disease, including those:

  • Age 65 years and older

  • Age 18 years and older residing in a long-term care facility, or

  • Age 50 through 64 years with underlying medical conditions or at increased risk of social inequities.


Among recipients of the 
Moderna COVID-19 vaccine, those who may receive a booster dose of COVID-19 vaccine include persons:

  • Age 18 through 49 years with underlying medical conditions or at increased risk of social inequities, or

  • Age 18 through 64 years who are risk for SARS-CoV-2 exposure and transmission because of occupational or institutional setting.

Janssen Info:

Janssen 
(Johnson & Johnson) Booster doses may be administered to those who received their first dose greater than or equal to 2 months ago.  
 
All those All those age 18 and older who received the Janssen (Johnson & Johnson) COVID-19 vaccine should receive a booster dose of COVID-19 vaccine.

If you have lost your CDC Vaccination Card, you may retrieve proof of vaccination through Washington’s MyIR (https://myirmobile.com/).

Please wear clothing that allows easy access to your upper arm. 
 

 

You should not get the vaccine if you:

  • Had a severe allergic reaction after a previous dose of this vaccine

  • Had a severe allergic reaction to any ingredient of this vaccine 


Tell the vaccination provider about all your medical conditions, including if you:

  • Have any allergies

  • Have had myocarditis (inflammation of the heart muscle) or pericarditis (inflammation of the lining outside the heart)

  • Have a fever

  • Have a bleeding disorder or are on a blood thinner

  • Are immunocompromised or are on a medicine that affects your immune system

  • Are immunocompromised or are on a medicine that affects your immune system

  • Have received another COVID-19 vaccine

  • Have ever fainted in association with an injection
