To schedule an appointment, do the following:
- Under the Enrollment Location select by product type (Medicare or Individual), and enrollment location (whether onsite at CHP or remote via telephone with a licensed agent).
- Under Appointment Type, select whether enrolling as a new member or a policy renewal.
All ONSITE appointments will be located at Capital Health Plan's Metropolitan building, located at: 1264 Metropolitan Blvd; we are located on the third floor.
