Scheduling Instructions 

1. Select an available date on the calendar and then click "Book It" to select your appointment time.
   • Click "Next" at the bottom to see additional available appointment times. 
2. Enter the required fields. 
3. Enter optional fields-  
   • Cell Phone: A text message appointment reminder will be sent to your cell phone an hour before your appointment.
4. Click, "Finalize Appointment".
5. An appointment confirmation will be sent to the email you provide.
6. Call  855-562-7821 at your scheduled appointment time.
