Before you arrive, please complete, download, and print the intake form: 13614-C Intake Form, or be sure to check in 15 minutes before the appointment to allow time for the intake and interview process
Bring to appointment
· Completed intake form
· Current photo IDs (you and your spouse, if any)
· Social Security cards for all persons on the return
· All applicable tax documents, such as:
· W2, 1099R, 1099SA, 1095A, Social Security
· Any letter received from the IRS, such as
· IP PIN after identity theft
· If direct deposit is desired, please bring a check or account verification
letter from the bank with the routing and account number
Schedule C filers (self-employed) must have
· Tallied (in writing) summary of expenses
· Mileage log entries
· Documents supporting income received
If filing a joint return, both spouses must be present to sign and submit your return.
Other important information:
- We are currently not preparing any returns with Bitcoin or any form of Virtual Currency.

- If you have already filed your tax return and are concerned about the status of your refund, please check the IRS Where’s My Refund webpage. After a return is transmitted and accepted, the tax site cannot track your refund.
- If you file jointly with a spouse, you must not create separate appointments.
- If you must file for multiple years, do not create an appointment for each tax year. Specify in the notes or contact the site with which years.
- All duplicate appointments will be removed. Failure to follow the procedure may cause you to lose your appointment.
