1.  Please check in 15 minutes prior to appointment

·         To complete paperwork for intake and interview process

2.  Prior to arrival, complete, download and print intake form. 13614-C Intake Form

3.  Bring to appointment

·       Completed intake form – if you have downloaded it

·       Current photo ID’s  (you and your spouse, if any)

·       Social Security cards for all persons on the return

·       All applicable tax documents, such as:

ü W2

ü 1099 R

ü 1099 SA

ü 1095 A 

ü Social Security

·         Any letter received from IRS, such as:

ü  IP PIN after identity theft

·         IF direct deposit is desired

ü  Bring a check or printed letter from the bank with routing and account number

4.   Schedule C filers (self-employed) should bring

·       Tallied summary of expenses

·       Mileage log entries

·       Documents supporting income received

Please note: If filing a joint return, both spouses must be present to sign and submit your return.

  • We are currently not performing any returns with Bitcoin or any form of Virtual Currency
