Welcome to Foundation Communities' Health Coverage Program!
Our expert team is available by appointment, year-round to help determine your eligibility to enroll outside of the November 1 through December 15 Open Enrollment period.
We are also available to assist you with navigating your coverage and care including but not limited to finding a doctor, reviewing claims/bills, explanation of benefits..... etc.
We offer phone appointments to Clients who face geographic or medical barriers to attend an in person appointment. To request a phone appointment please call 512-381-4520.

Instructions for Consumer: Please bring the following documents with you:
- Username and password for healthcare.gov, if you already have an account
- Big envelope we gave you, if you have visited us before
- Immigration documents for everyone applying for insurance
- Cost & details of insurance offered by your or your spouse's employer
- List of your doctors and medications (name & dosage)
